About Us

This website is part of a project, called "Destination Known: Making Health Care Transition Happen for Youth with Special Health Care Needs in the District of Columbia." Our aim is to reduce barriers to successful health care transition from pediatric to adult health care systems. This involves helping teens and families get ready for transition to adult health care, assisting teens in becoming more independent health care consumers, making available useful resource materials on transition, partnering with health care providers in making a smooth transfer to adult health care, and developing policies that facilitate transition.

The health section of this website is a collaborative project of Partners in Transition, Family Voices-DC, and the National Alliance to Advance Adolescent Health, and is funded by DC’s Community Health Administration.

In addition to the website, the National Alliance’s project activities include a focus group study of adolescents with special needs and parents whose teens have special needs, an analysis of DC transition data from the National Survey of Children with Special Health Care Needs, a survey of DC physicians, a year-long transition quality improvement project in DC with three pediatric and two adult primary care practices, teen and family mentoring opportunities, and physician training.
